ADAMS Compassionate Healthcare Network (ACHN)

Founded in 2013, ACHN is dedicated to providing medical services to low-income and uninsured individuals. As our name suggests, we provide compassionate, quality healthcare to the most vulnerable of our population for free or at low cost. Our comprehensive services are made possible thanks to the dedication and time of our community health partners and volunteer network of over 80 medical volunteers which include 15 primary care physicians, 32 specialists, and 33 medical volunteers, assistants, and student trainees.

Our innovative healthcare model is available to all low-income and uninsured individuals who qualify regardless of race, religion, ethnicity, or zipcode.

What we do

Thanks to our partners, volunteers, and donors, we provide a wide range of patient services including primary and eye care, preventative screenings, blood work, imaging, prescriptions, and referrals. Our integrated approach also includes hot-spotting, telehealth services, care coordination, food and housing security evaluations, and preventative health education.

ACHN patients are seen by appointment only. All patients must qualify and register to enable the us to provide the highest quality of integrated care. Registration helps our healthcare providers keep track of patient history which ensures an optimal patient treatment approach.

Who we serve

Throughout our years of operation-one truth has remained constant: the continuing need for essential health care services for the most vulnerable populations. ACHN is deeply committed to expanding access to high quality care for those who need it most.

This is why we serve eligible patients from all areas, regardless of zipcode and state. To be eligible, patients must be uninsured and low-income, with all patients living at or below 200% of the Federal Poverty Level. Income verification is conducted during the patient registration process.
