ACHN primary care services are free! We offer a range of primary care services such as preventative screenings, routine healthcare check-ups, well-woman exams, prescribing and dispensing medicine and more.

In order to register, eligibility must be met. You may qualify for ACHN’s free consultations and discounted services if:

You do not have medical insurance AND your household income falls within 300% of the Federal Poverty Level OR you receive Federal or State benefits, such as: Food stamps (SNAP), VA TANF (Temporary Assistance for Needy Families), Section 8 Housing, Unemployment benefits, Social Security benefits, School Free Lunch Program.

If eligibility is met, one can register by filling out the eligibility forms listed on our website. Bring this paperwork to the clinic to be processed.

Yes, we offer referral services for Cardiology, Ophthalmology, Gastroenterology, Mammography and Dermatology. We also have services with a Retina specialist and an Orthopedic Hand Surgeon.

After completing your lab, your lab results will be sent to ACHN and your provider will read the results. Once the provider has read them, you can view your results in the Patient Portal or you can come in to receive a print-out copy of your lab results.

If you are more than 15 minutes late to your appointment, you will not be seen and must reschedule. You will also be charged $10 penalty at your next office visit.

You must cancel your appointment by calling our office at least 48 hours in advance. Any cancellation less than 48 hours will result in a $10 penalty at your next office visit.

If you do not show up for your appointment, you will be charged a $10 penalty at your next office visit. After 3 consecutive no shows, you will lose your eligibility.

All blood tests are either performed at ACHN or at Sunrise Medical Laboratories only.  If your blood test was not done at ACHN, it must be preformed at Sunrise Medical Laboratories only, If you decide to go to another lab, ACHN will not be responsible and will not serve as a medicator. Therefore, be sure your lab test is done at ACHN or at Sunrise Medical Laboratories only.

ACHN is open on Monday, Tuesday, and Wednesday from 9:00am-3:00pm or on Saturday from 9:00am-12:00pm.

ACHN does not pay for hospital or any other emergency medical care. ACHN is not an insurance company.

ACHN does not offer dental services, but provide list of low-cost options for dental services.
